Social Work Co-Manager
Part time 24 hours per week.  The Social Work Manager oversees the implementation of the Social Work Program within the agency.  Responsible for planning and implementing the in-service education and competency training for the Social Work staff.  Reports directly to the CEO. Masters degree in Social Work from a school accredited by the Council in Social Work Education. Currently licensed as a Licensed Independent Clinical Social Worker in Rhode Island and Massachusetts. Minimum of four years experience in social work with a minimum of two years in home care.  Two years in a supervisory position.

Hospice Social Worker
Part time 32 hours per week.  The Hospice Social Worker is responsible for the initial and continuing assessment of patient/family psychosocial needs and implementation of interventions in accordance with the interdisciplinary team plan of care to assist the patient/family in dealing with the psychosocial aspects of terminal illness.  Works under the supervision of the Social Work Manager and the Hospice Director. Masters level graduate of a school of social work accredited by the Council on Social Work Education. Currently licensed as Clinical Social Worker in the states of Rhode Island and Massachusetts. Minimum of one year experience in medical social work preferably in a home care setting. Hospice training/experience preferred.

Registered Nurse – Homecare
Full time, part time and per diem positions available serving Providence, Kent, Newport and Bristol Counties. Provides skilled professional nursing care in the home care setting as prescribed by the physician. Gives care in a manner that promotes quality, continuity and safety for the patient and family. Must be registered as a professional nurse in Rhode Island. Massachusetts license must be obtained within one year of employment.

Registered Nurse – Hospice
Full time, part time and per diem positions available serving Providence, Kent, Newport and Bristol Counties. Provides skilled professional nursing care in the home care setting in accordance with the National Association of Hospice and Palliative Care standards and as prescribed by the physician. Gives care in a manner that promotes quality, continuity and safety for the patient and family. Must be registered as a professional nurse in Rhode Island.

Certified Nursing Assistant
Full time, part time and per diem positions available serving Providence, Kent, Newport and Bristol Counties. Assists patient with personal care according to the plan of care developed by the case manager following state and agency guidelines. Observes, records and reports vital signs and other data as requested in the care plan. Assists patient with ambulation and transfers when appropriate. Performs homemaking services that are essential to the patient’s care at home. Must be registered as a CNA in Rhode Island.  Massachusetts license must be obtained within one year of employment.

Physical Therapist
Full time, part time and per diem positions available serving Providence, Kent, Newport and Bristol Counties. Provides direct physical therapy services to patients in the home care setting. Determines priority needs for physical therapy services depending on potential for benefit, diagnostic category, age and chronicity of condition. Provides physical therapy evaluation and instruction to nursing staff. Must be registered as a professional Physical Therapist in Rhode Island. Massachusetts licensure must be obtained within one year of employment.

Occupational Therapist
Full time, part time and per diem positions available serving Providence, Kent, Newport and Bristol Counties. Provides occupational therapy services to patients in the home care setting. Determines priority needs for occupational therapy services depending on potential for benefit, diagnostic category, age and chronicity of condition. Provides occupational therapy evaluation and instruction to nursing staff. Must be registered as a professional Occupational Therapist in Rhode Island. Massachusetts licensure must be obtained within one year of employment.

Maternal/Child Health Manager

Part time 24 hours. The Maternal Child Health manager is responsible for the management and supervision of the Visiting Nurse Home & Hospice Maternal Child Health Program and Visiting Nurse Home & Hospice Pediatric Care Program per agency mission / vision to ensure high quality care.  Serves as a community and state liaison for maternal/child health activities and collaborates closely with the RI Department of Health and other resources to identify families appropriate for First Connections.   Reports to the Director of Homecare Nursing.

Community Liaison

Full time.  Responsible for developing and maintaining good relationships with referral sources.  Facilitates/coordinates patient discharges to Visiting Nurse Home & Hospice Homecare services.  Promotes Agency services to referral sources and community.  Reports to the Director of Business Development.
