RN Case Managers
Visiting Nurse Home & Hospice is seeking experienced RN Case Managers for our growing Home Care and Hospice divisions. Social Work Co-Manager
Part time 24 hours per week. The Social Work Manager oversees the implementation of the Social Work Program within the agency. Responsible for planning and implementing the in-service education and competency training for the Social Work staff. Reports directly to the CEO. Masters degree in Social Work from a school accredited by the Council in Social Work Education. Currently licensed as a Licensed Independent Clinical Social Worker in Rhode Island and Massachusetts. Minimum of four years experience in social work with a minimum of two years in home care. Two years in a supervisory position.
Maternal and Child Health Program Coordinator
Part time 24 hours per week in the Portsmouth office. The MCHP Coordinator assists the MCH Manager with the management and delivery of the Maternal and Child Health Program. Provides direct patient care. Works under the direction of the MCH Manager. Serves as a supervisor, role model, and consultant to the MCH per case staff. The Coordinator collaborates with hospital nursing staff, physicians and other primary care providers to provide assessment and continuity of care as needed for health teaching and skilled care. Licensed as a professional nurse in Rhode Island; Massachusetts license must be obtained within one year of employment.
Maternal and Child Health Program Outreach Worker
Per diem position. The Maternal and Child Health Outreach Worker is responsible for assisting the MCH and First Connections team members in locating and contacting families and identifying and promoting community services appropriate to each individual and family member (birth through 36 months) referred to the First Connections Program. Will assist and collaborate with team members by providing community resource information as needed within the First Connections program. Position requires minimum 1 year experience. Associates degree in human services or related field or Certified Outreach Worker certificate. Strong knowledge of state and community resources. Reports to the Maternal & Child Health Program Manager.
Hospice Social Worker
Part time 32 hours per week. The Hospice Social Worker is responsible for the initial and continuing assessment of patient/family psychosocial needs and implementation of interventions in accordance with the interdisciplinary team plan of care to assist the patient/family in dealing with the psychosocial aspects of terminal illness. Works under the supervision of the Social Work Manager and the Hospice Director. Masters level graduate of a school of social work accredited by the Council on Social Work Education. Currently licensed as Clinical Social Worker in the states of Rhode Island and Massachusetts. Minimum of one year experience in medical social work preferably in a home care setting. Hospice training/experience preferred.
Certified Nursing Assistant
Full time, part time and per diem positions available serving Providence, Kent, Newport and Bristol Counties. Assists patient with personal care according to the plan of care developed by the case manager following state and agency guidelines. Observes, records and reports vital signs and other data as requested in the care plan. Assists patient with ambulation and transfers when appropriate. Performs homemaking services that are essential to the patient’s care at home. Must be registered as a CNA in Rhode Island. Massachusetts license must be obtained within one year of employment.
Clinical Reimbursement and Authorization Assostant
Per diem position assisting with day to day operations of the Clinical Reimbursement and Authorization Department, reporting to Department Director. Verifies authorization for admissions, reviews weekly and follows up with clinicians on point of care for more authorizations or discharges from services. High school diploma or equivalency required, with medical coding and billing certification preferred.
