RN Case Managers
Visiting Nurse Home & Hospice is seeking experienced RN Case Managers for our growing Home Care and Hospice divisions.  Our innovative organization focuses on patient-centered care, utilizing the newest technologies to deliver the highest of quality.  Our Home Care ranks in the top 20% nationally, and Hospice in the top 5%!  Generous benefit package.  Positions available in Kent, Providence, and Newport Counties.

Certified Nursing Assistant
Full time, part time and per diem positions available serving Providence, Kent, Newport and Bristol Counties. Assists patient with personal care according to the plan of care developed by the case manager following state and agency guidelines. Observes, records and reports vital signs and other data as requested in the care plan. Assists patient with ambulation and transfers when appropriate. Performs homemaking services that are essential to the patient’s care at home. Must be registered as a CNA in Rhode Island.  Massachusetts license must be obtained within one year of employment.

Interpreter
Per diem position.  Under the general supervision of the Director of Homecare Nursing, provides interpretation between non English-speaking patients and their health care provider to assure quality of care.  Translates accurate communications between health care provider and patients.  Translates written documents.  Explains functions of the care provider and procedures to be expected to patients.  Questions patients to gather necessary information for all records and documents. Interprets for incoming calls as necessary and available.  High school graduate or equivalent.  Must have valid driver’s license with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
